The clinical problem
Prescribing has moved faster than the gut advice
GLP-1 receptor agonists slow gastric emptying and small intestinal transit. That slowing is a large part of how they produce satiety and glycaemic control, so the gut consequences come from the same mechanism as the benefit. It is also why they rarely settle on their own.
Your clients are not taking bloating and early fullness back to the prescriber. They are bringing it to you. And "your medication is doing that", however true, is not a plan.
Stasis is the precondition small intestinal overgrowth needs. A client on months of slowed transit, falling fibre intake and shrinking dietary variety is a client whose microbiome is being reshaped while nobody is watching it.

The pre-treatment work-up: what to screen for, why constipation gets resolved first, how to build fibre and diversity while appetite is still intact, the therapies worth having in place, and nausea support.
3 · The SIBO risk
Why the mechanism that makes these drugs effective is the same one that predisposes to overgrowth. What to monitor, when testing is worth it, and how to support motility through treatment rather than cleaning up afterwards.
